Incident response

This flow creates a critical ticket, enumerates project members, sends an organization notification to up to 100 recipients, records an incident-management comment, and moves the ticket into active work.

Production variants should add a stable incident identifier, avoid notifying unrelated members, and record links to the monitoring/incident system only as safe internal application paths where the notification contract requires /app URLs.
